We don't want to sugarcoat it. Finding resources for yourself or loved ones for addiction is not easy. We assess what level of care you need to get you started with your recovery.

What is a Chemical Health Assessment?

Our assessments can satisfy probation requirements and DUI violations. It can also address employer or personal concerns. 

How long does it take?

Your assessment takes 60-90 minutes. Your clinician will then follow up with you 7-10 days after you meet with recommendations.

What does it cost and who do I meet with?

Our Chemical Health Assessments are billed through insurance, please talk with your clinician to confirm you are in network. Assessments are completed by our Licensed Alcohol and Drug Counselors (LADC), Melissa Fjelstad and Julia Edelman.

Will I be drug tested? 

Lakeshore does not offer urine drug screenings. 

How long is it good for?

Chemical Health Assessments are valid for 30 days. 

Is this a Rule 25 Assessment?

A Chemical Health Assessment is different from a rule 25. If you are looking for a rule 25 assessment a Chemical Health Assessment will not meet that requirement.
